What is the best way to set up an appointment?

Feel free to call, email, or fill out the information form on the right-hand side of this page. I will typically get back to you within within 2-3 business days.

This is my first appointment, what should I bring and what do I wear?

Please wear comfortable clothing or feel free to bring a change of clothes.

Prior to your first appointment, please fill out the forms listed below. You can print them out and bring them to the session or you can fill them out during the initial visit. Filling them out ahead of time will expedite the process:

What will take place at the first appointment?

We will start with a discussion about the areas of challenge (subjective history) followed by a thorough evaluation of your biomechanical system and function. A treatment plan will begin to be formed. Often, treatment will begin toward the end of the first visit.

How is progress going to be measured?

Objective measurements of strength, range of motion, flexibility, joint mobility and other metrics are taken at the first visit and again at future visits to track progress; subjective measurements including pain range and function are also included in areas that are tracked.

Will you communicate with my physician?

An initial evaluation report with your plan of care will be written and sent to your physician. Your physician will sign and return the evaluation report if they agree with your plan of care and this will serve as your prescription for physical therapy. You only need a prescription for physical therapy after being treated for 30 days. Re-evaluation reports will also be sent to your physician to communicate your progress.

How long does physical therapy last?

Physical therapy is different for every person and since we treat the individual, not the symptoms, it really does vary. We’ve had patients that have only needed a few sessions to get back on the right track; We’ve had patients that have seen success over the course of a few months; and We've had patients that see me on a recurring basis for as long as we’re still seeing progress.

Are your services covered by insurance?

Essential Physical Therapy is not a participating provider with any insurance company. This is due to the fact that Essential Physical Therapy would not be able to spend the time necessary to provide high-quality, personalized care at the rate insurance companies are currently willing to reimburse for physical therapy.

Can I submit this to my insurance company?

In most cases, you can submit for out-of-network reimbursement through your insurance company. There are forms available on your insurance company’s website; you will need to fill out that claim form and send it in with the superbill from your physical therapy visit (which will be provided upon request). The amount of reimbursement depends on each individual’s benefits. You may also choose to call your insurance company to discuss “out-of-network” physical therapy coverage.
